a Home Visit
We encourage all patients
to attend centre, but if you feel you cannot and may
need a home visit call 01244 357800 or 01244 881590
before 10.30am. You will be asked some details of your
problem and may be contacted by a health care worker
to discuss further.
Obtain
help when the Surgery is closed
There is an 'Out of Hours' (OOH) service for urgent
problems run by the 'Primary Care Trust' (PCT) in the
evenings and weekends. If you have a problem that cannot
wait until the next day please telephone 01244 343300;
where you will be put through to the Out of Hours Service.
Shortly after the Easter bank hoilday weekend the telephone
number for Out of Hours will be changing to 01244 385300.
Book a Routine Appointment outside normal surgery

Western Cheshire PCT
have launched a new centralised booking service. This
service benefits patients who wish to have an appointment
outside the hours that the practice is open.
The clinical appointments
for the centralised service are held in 1829 Building
at the Countess of Chester Health Park, where patients
can book a routine appointment with a GP or
Nurse during the hours of 6.30pm to 8.00pm
Monday to Friday and on a Saturday from 10.00am to
12.00pm. The appointments can be made by telephoning
01244 385422 between the hours of 8.00am to 6.00pm
Monday to Friday.

Results
Please telephone 01244 357800 at City Walls Medical
Centre between 2pm - 4 pm for all results.
Leave a specimen for a test to be carried out
If they are correctly labeled and have a completed
form they can be left in the basket on the front reception
desk. Otherwise please see the Practice Nurse.
Obtain a Sickness Certificate
A sickness certificate completed and signed by your
doctor is only required when a period of continuous
absence from work due to illness is longer than 6 days.
Periods of absence less than 3 days do not need a certificate.
Between 3-6 days a self-certification note can be completed
using form SC1 available at reception.
Please remember you will have to see a doctor if you
wish to renew a sick note or return to work before
your
sick note expires.

The Surgery
supports Carers. We have 2 dedicated members of staff
who attend
meeting 4 times a year so that we are up to date with
all current carers information. Please see the Carers
information board in the reception area at both Practices.
To make a
Complaint
If you feel the need
to make a complaint please speak to the Receptionist
who will try and resolve your problem immediately.
If you remain dissatisfied and wish to make a formal
complaint please ask to speak to the Practice Manager,
Office Manager of Head Receptionist. If this is a medication
complaint, please ask to speak to the Medicines Manager.
Make a Suggestion
If you would like
to make a suggestion regarding the Practice or it's
services, please put it in writing addressed to the
Practice Manager.
